1. PURPOSE. This Policy sets forth the terms governing the grant of maternity leave benefits to qualified female employees of ACME Corp Holdings, Inc. in accordance with Republic Act No. 11210 and its Implementing Rules and Regulations.
2. COVERAGE. This Policy shall apply to all female employees regardless of civil status, the legitimacy of her child, employment status, and length of service, who are qualified SSS members and have remitted at least three (3) monthly contributions within the twelve-month period.
3. ENTITLEMENT. A qualified female worker shall be entitled to maternity leave of one hundred five (105) days with full pay regardless of mode of delivery, with an additional fifteen (15) days for solo parents as defined under R.A. No. 8972.
4. PROCEDURE. The female employee shall notify her immediate supervisor of her pregnancy and the probable date of her childbirth at least thirty (30) days in advance, transmitted to HR for processing of the SSS MAT-1 Notification Form.
5. ALLOCATION. A qualified female worker may, at her option, allocate up to seven (7) days of said benefits to the child's father, whether or not married, subject to written notice to both employers.
6. EXTENSION. A female worker may, at her option, extend her maternity leave for an additional thirty (30) days without pay; provided that the employer shall be given due notice...
